What to Expect at your Consultation

Drs. Perrotta and Luppens will devote as much time as necessary to discuss your individual goals and needs with you. After a focused physical examination, your surgeon will provide preliminary recommendations and explain what you can expect from surgery.

Once your surgeon determines his surgical plan, our aesthetic coordinator will produce a written quote, which lists all costs of your proposed surgery so that there are no surprises. At that time, an appointment is set for your pre-op visit. 

Pre-Op Visit

Much is covered during your pre-op visit. First, one of our highly trained medical assistants will take pre-op photos. Next, your surgeon will explain the surgical technique to you and thoroughly inform you as to what you can expect, including any potential complications. Note that all procedures involve some risk, but major complications are rare. Dr. Perrotta or Dr. Luppens will then obtain a more complete medical history from you and perform a final pre-op physical examination, including a check of your vital functions to ensure that you are physically fit for surgery. All informed consents for surgery will be filled out at this appointment and final payments are due at this time.  You will be notified 24 hours prior to your surgery with your arrival time.

The Tummy Tuck Procedure

Dr. Perrotta and Dr. Luppens perform tummy tuck surgery with IV sedation and local anesthesia or general anesthesia. Younger and healthier patients are able to go home on the same day of surgery, while those with significant surgical risks stay overnight in the hospital.

During the surgery, excess skin is removed, and the deep muscle layer is tightened. Liposuction to remove excess fat is usually performed simultaneously. Your surgeon will make the incisions along the pubic hair line and extend toward the lower waistline, within the bikini line, where they are less noticeable. Occasionally, when there is a large amount of excess skin in the horizontal direction, a vertical incision must be made in addition to the lower, horizontal incision.

Post-Op Recovery

You will be asked to keep your hips and knees flexed at all times during the first week or two after surgery. You will also probably need to wear a specialized post-operative garment to control swelling and/or protect the deep muscle repair. You will be able to walk around the house the day after surgery and resume light activities within 3-7 days.

If your tummy tuck procedure involved skin tightening only, you can resume restricted work within 7-10 days and full work, including even strenuous physical activity, in approximately 2-3 weeks. If your muscles were also tightened during your surgery, you must refrain from strenuous physical activity for 4-6 weeks. After your procedure, you will follow up with us several times so that we can monitor your healing.

Mini Tummy Tuck

Excess skin is addressed by removal. For many patients, this is all that is necessary to achieve a slimmer look. The technique is known as a “skin only” or “mini” tummy tuck and can be used to tighten loose skin and remove stretch marks located below the belly button, as well as all poor quality skin above and below the belly button.

Mini Tummy Tuck Procedure

Excess skin is tucked and removed through an incision that runs horizontally and below the underpants or bikini line. The advantages of a “skin only” tummy tuck include less discomfort during the recovery period because no muscle tightening is involved. For most patients, the muscle tightening is what causes most of their discomfort after the procedure. Also, liposuction can be used to do more contour correcting, when necessary, because not as much work is being done under the skin.

FAQ’s

For those with loose or sagging skin on their abdomen, a bulging abdomen due to muscle separation, or those who can’t tighten the skin through exercise or diet.

Initial results are typically visible once the swelling subsides, usually within a few weeks to a few months after surgery. However, final results may take several months as the body heals and adjusts.

Long-lasting results are achieved, but aging, weight fluctuations, and lifestyle choices can shorten their lifespan.

Before the procedure, follow any pre-operative instructions provided by your surgeon, which may include dietary restrictions and cessation of certain medications. After the procedure, follow all post-operative instructions provided by your surgeon, which may include wearing compression garments, avoiding strenuous activities, and attending follow-up appointments.

A tummy tuck typically involves downtime, and common symptoms include swelling, bruising, discomfort, and temporary numbness in the treated area. The surgeon discusses the risks before the procedure.

Tummy tucks involve removing excess skin, tightening the muscle layer, and liposuction to remove excess fat. It requires IV sedation and anesthesia, and incisions are made within the bikini line to minimize visibility.

While the majority of the information here can be applied to a tummy tuck performed anywhere, some of the information applies specifically to practices of Dr. Perrotta and Dr. Luppens of Peninsula Plastic Surgery. Speed and quality of recovery depend on the type of abdominoplasty. Skin-only abdominoplasty patients recover faster than full abdominoplasty patients. During a skin-only abdominoplasty, the surgeon removes the excess skin but does not tighten the deep muscle layer. In a full abdominoplasty, he also tightens the deep muscle layer. In the Recovery Room–the moment you wake up from anesthesia, you will notice the post-op garment surrounding your belly. It will feel very tight.  You will also notice one to three soft rubber tubes with clear bulbs on their ends. The nursing staff will teach you and your caregiver how to manage these drains (see below). You will feel drowsy and as if you keep falling in and out of sleep. You might feel some nausea. Both of these sensations are from the anesthesia.

First Night After Surgery – You will feel drowsy. As the anesthesia wears off, you will start to feel pain. You may also feel nausea, due to the anesthesia and pain medication. Your pain medication can also make you feel itchy. If you develop a rash or hives, call your doctor immediately. You or your caregiver will have to empty the drain bulbs every four hours.  If any of them fill up in less than four hours, call your doctor and discuss it with him. For directions on emptying your drain bulbs. Never disconnect the bulb from the drain. Remember to take your antibiotics, keep your hips flexed at all times, and flex your ankles at least twenty times every hour to prevent deep clots from forming in your legs. If you begin to vomit or retch, call your doctor so that he can call in some anti-nausea medication.  Retching and vomiting can jeopardize the repair on your deep abdominal wall and cause more bleeding. If you underwent a full abdominoplasty, avoid using your abdominal muscles. To rise up in bed, roll over to your side first and then push yourself upward with your hands.  When entering your bed, crawl in front first and then roll to your side. To sit up, again, use your arms, not your belly muscles. When you walk, do so bent over to reduce the tension across the suture line.  Skin-only abdominoplasty patients can ignore this paragraph.

Day after Surgery  – You will be more alert. You might notice more pain as you become more mobile. Also, you will notice the onset swelling. Make sure you are drinking plenty of fluids

Second Day after Surgery – You will experience more pain on this day than any other, and you might see swelling of your ankles and feet. Both are due to the inflammation and edema (fluid collection). Toward the second half of this day, you will start to visit the bathroom very often to urinate.

Day 3 through Day 7 – You will continue to gain function. Your requirement for narcotic pain medication should taper off. Your drainage should diminish enough for your tube(s) to be removed at the doctor’s office. Between day five and seven, you should finish using narcotic pain medication and switch over to Ibuprofen or Tylenol.

Day 8 through Day 14 – You are gaining function and walking more upright.  You will feel occasional sharp pains when you twist and produce some internal tension along the internal fibrosis (scarring).   Full abdominoplasty patients must refrain from lifting anything over 10 pounds, as straining can destroy your deep wall repair.  Since you no longer are using narcotic pain medication, you should be able to drive.

Day 15 through Day 28 – Skin-only abdominoplasty patients should be ready to return to full activity, including work. Full abdominoplasty patients must continue to  avoid lifting objects over 20 pounds and may start walking exercises but nothing strenuous enough to cause straining. These patients with desk jobs can return to work, provided they continue to honor the restriction on heavy lifting.

Day 29 through 42 – Some time during this interval, surgeons will inform full abdominoplasty patients they are fully recovered and can return to full function, including strenuous physical activity.
